Parent/Student Surf Team Application Form

The following is instructions for using the online form.

1. Save the Parent/Student Surf Team Application to your Desktop.
2. Open Microsoft Word and click File>Open. Locate the Parent Form on your desktop and click Open.
3. With the form open, follow the instructions to fill out all pages. You will type in the highlighted fields in the form. Do not print this form.
4. Once you have completed filling out the form by typing in all necessary information and checking all of the approptiate boxes, click Save to save your file.
5. Email the form back to your Surf Coach by attaching the file to the email. Once this form has been received by your coach, and the fees have been paid, your child will officially be in the surf club/team.

The following form is an online form. Save the form to your computer, open it in Microsoft Word, type in the information, save it, and email it back to Kim Clary.

First Aid/CPR Training

First Aid and CPR training are a pre-requisite for all Head Coaches. The CSSL highly recommends all of the Assistant Coaches be certified and up to date with their certifications as well.
